To add a question to a Web Form, select the "Questions" option from the Web Form listing. The page that appears will display all pages and questions on a single page. To add a question above an existing question, click the "Insert" link beside the existing question. To add a question to the bottom of the page, click the "Insert Question" link after the last question on the page that you want to add the question to.

The next page you see will be a list of available question types. After selecting a question type, you will be taken to a form that has different fields, depending on the type of question chosen.

The question types are (click for additional details):

  2. Text - Single Link
  3. Text - Multiple Lines
  4. Radio Buttons
  5. Radio Button Group
  6. Single Select Drop-Down
  7. Multiple Select Drop-Down
  8. Check Boxes/Custom Boxes
  9. Date/Time
  10. Document

The following fields are used for each type of question. To view details about fields that are specific to the different question types, click the links in the previous list.

General

1. Field Type
 
This is the type of question that is currently being added. If you change this value before the question is complete, the form will reload with the options that are specific to the type of question you change the value to.
2. Caption
 
This is the actual question as the user will see it. Users of Internet Explorer 5.5 or higher on the Windows platform will have access to a WYSIWYG HTML editor; others will have access to a standard text field for entering the caption.
3. Short Name
 
The Short Name is used for list headings, as well as to store data within the Web Form database. For example, if your question is What is your first name?, a good Short Name would be First Name.
 
Note: The following fields apply to all question types except the "Heading" type.
 
7. Required
 
If a question is required, users will receive an error message when submitting the form if they do not provide an answer.
8. Public Read
 
This defines whether or not a user (who is not a form manager) can see this question and its answer when viewing a submission.
9. Public Write
 
This defines whether or not a user can see the question when filling out the form.
10. Placement
 
This defines whether the field of the question appears on a new line under the caption or immediately to the right of the caption.

11. Separator
 
This defines whether or not a blank row will separate this question from the next one.

Alignment

1. Caption - Vertical
 
Specifies the vertical placement of the question caption in relation to the question field. Possible choices are top, middle, and bottom.
2. Caption - Horizontal
 
Specifies the horizontal placement of the question caption in relation to the question field. Possible choices are left, center, and right.
3. Field - Vertical
 
Specifies the vertical placement of the question field in relation to the question caption. Possible choices are top, middle, and bottom.
4. Field - Horizontal
 
Specifies the horizontal placement of the question field in relation to the question caption. Possible choices are left, center, and right.
